This is version 1.3 of the draft Maul rules. It has a few subtle changes from the prior version.
Please note that this rule is still being play-tested. Any suggestions/criticisms appreciated.

ATTACKING SIDE READ THIS...
CREATING A MAUL
A coach can create a maul at any time during his turn if he has a player with the ball and one other team mate in any of the 3 tackle zones behind him. To create the maul simply state to your opposition that a Maul is now created.
Once the maul is created the ball carrier may not be tackle until the maul has ended.
ENDING THE MAUL
The maul is ended when the ball is removed from the Maul or the Maul collapses.
MOVING THE MAUL
Once the Maul is created it can be moved forward. The Maul will always move directly forward in a straight line towards your opponents try-line. To move the Maul forward you must use more team mates to bulk it up and push it forward.
For every 2 extra players you add to the maul, the Maul will move forward 1 space.
When adding more players to the maul start by filling the free tackle zones first, any other players can be added directly behind those - one row at a time.
Once a player has joined the maul he may not leave until the Maul has ended.
COLLIDING WITH OPPOSITION PLAYERS
If when rumbling forward any opposition players, directly in the path of the maul, touch the maul then those players are considered to become part of the maul and will move backwards with the maul.
GATHERING THE BALL FROM THE MAUL
The Ball can be Gathered from the Maul at any time during the controlling sides turn. Any player at the back of the Maul may Gather the Ball by simply making a Ball Skill roll. If successful the ball is placed on this player and he is free to move as normal. The Maul is now considered over.
If the dice roll is failed your turn ends immediately.
DOUBLE 6
If you roll a double 6 when Gathering then the ball is spilled and knocked on. Your opposition are awarded a scrum.
COLLAPSING THE MAUL
The side controlling the maul can choose to collapse the maul at any time. Simply turn over every player involved in the maul and continue playing as normal. The maul is now considered a ruck.
DEFENDING SIDE READ THIS.
MOMENTUM/CONTROLLING
The team that starts the maul will always have the initial momentum and for that reason they will be the only team allowed to Move the maul and Gather the ball. However the opposition can in their turn attempt to switch the momentum by mustering up a Counter Shove!
Shoving an opposition maul backwards is no easy task and demands a huge amount of power and aggression. To Counter Shove your opposition you must do the following...
1) Count up how many of your players you have involved in the Maul (add extra players to increase your chances).
2) Roll equal to or less then this number...
If the roll is successful then you have managed to switch the momentum. With the momentum switched it means YOU now control the Maul and can now Move it or Gather from it. Every 2 extra players you now put into the maul will move it forward 1 space toward the attacking teams tryline.
If you fail the Counter Shove roll your turn ends immediately.
DOUBLE 6
If you roll a double 6 when attempting a Counter Shove, then you are deemed to be deliberately pulling down the maul. The Maul collapses and your opposition is awarded a penalty on the spot containing the ball
DELIBERATE COLLAPSING
The side without control of the Maul can choose to deliberately collapse it during their turn. To do so simply turn all players from both teams over to show that they are now grounded - the maul is now considered a ruck. Roll a referee roll. If successful, continue playing as normal. If you fail a penalty is awarded to your opposition on the spot containing the ball.

I don't think a maul can be formed without involving an
opponent so I suggest that when a player is about to
be tackled, if one of the ball carrier's mates is in
one of the 3 zones behind the carrier then the coach
can call for a maul instead of taking the tackle, all
players staying on their feet. Two questions: When is the
forward move applied and is it applied every turn?

It's more about the intention on forming a maul, players grip the ball carrier and provide support with the intention of mauling.
Movement happens as soon as the additional players are added.

You can form a maul without involving an opponent.When a player is coming back to the ball carrier and is then given the ball you are in a maul situation whether there is an opponent or not. (this is different from a player with the ball touching one of his front mate, that is not allowed).
Not opposing a maul awards the defending team the right to by pass it (turn around) and tackle immediatly the ball carrier. However this is quite difficult to do as the tackler has to be alone while the maul is progressing, and in "game" terms would require, I think, a roll.

In rugby, if the attacking team team has the momentum it can move the ball cimsiderable distances. What if, if the attacking team has the momentum it can move the ball with a series of dice rolls, untill the defensive side gets enough players in the maul.
Since forwards are involved: If you thow less or the same as the Skills of a forward you move 2spaces
If you throw more than the Skills chart the maul collapse and becomes a ruck and can only be played by the scrumhalve, if the scrumhalve is not there the defending side can counter ruck and create a turnover.
